The types of CLTC we provide
We provide "Community Long-Term Care for the State of South Carolina. These are a overall view of the types of care.
Personal Care
• Personal Care - Help with activities such as bathing, dressing, preparing meals, general housekeeping and observing health status
Companion Care
• Companion Services - Short-term relief for caregivers and/or needed socialization for participants.
Respite Care
• In-Home Respite Care - Temporary relief for the primary caregiver that keeps you or the participant at home and cared for.
HOW TO GET OUR SERVICES ? An individual or their family member may make a referral for CLTC services by calling the toll-free CLTC referral number, (888) 971-1637) or by using the CLTC website, https://phoenix.scdhhs.gov/eref/.
